Billy Forrester, a boy with a queasy stomach, has to move to a new town because of his father's new job. Billy dislikes the move and quickly misses his friends. When he goes to school, the bully Joe Guier and his friends fill his thermos of soup with worms. Joe laughs at Billy for having worms in his lunch, to which Billy replies, "I eat them all the time. You want to try some?" Billy then hits Joe in the face with a worm. As revenge, Joe and his friends make fun of Billy, calling him "Wormboy." Billy bluffs that he eats worms all the time, and ends up betting he can eat ten worms in one day or he'll stick worms in his pants and walk through school. During a single Saturday, Joe's friends prepare worms for Billy to eat, each method of preparation more disgusting than the last. Billy eats the last worm with seconds to spare, thereby winning the bet. Joe's older brother Nigel comes by and abuses Joe for losing the bet, which will make Nigel look bad to his own friends. Billy stands up to Nigel, who leaves. Because of a miscount, Billy realizes he only ate nine worms. Joe and Billy, now friends, both stick worms in their pants and walk through the school.

- In the book, Billy has to eat fifteen worms in fifteen days, and the terms of the bet are fifty dollars.
- Erika, a girl who helps Billy in the film, is not introduced until the book's sequel, How to Fight a Girl.
- Most of the characters in the book are not in the film, and vice versa.
- Instead of standing up to Joe's bullying, Billy wants the money to buy a motorbike.

- The Filthy Critic gave the film four out of five "fingers" for its realistic portrayal of how children really act.
- Rotten Tomatoes shows it as being rotten with a 58% rating.
